Located in a quiet area of the municipality of Barcelos, the 5Sensi unit is a structure that seeks to promote the well-being, comfort and quality of life of the users.

The family and friends of patients admitted have offered a Free Shuttle Service from the unit to the train station of Barroselas and train station of Barroselas to the Unit.

The Unit of Continued Care Integrated 5sensi (forward designated as Unit 5sensi) is inserted in National Network of Continued Care Integrated (RNCCI). The RNCCI was created by the Decree-law no. 101/2006, of 6 June, as part of the Ministries of Health and of Labor and Social Solidarity.

The provision of continued care is ensured by Units of Internment (as our Unit), Units of Outpatient, Hospital Teams and Home Visits Teams.
There are many types of units of internment, Unit of Convalescence, Unit of Average Duration and Rehabilitation Unit, Long-Term and Maintenance Unit of Palliative Care. Each of these types has its own characteristics that distinguish them.

The 5sensi is a Unit of Long Duration and Maintenance, and may take a temporary or permanent and is dedicated to the provision of social support and health care maintenance, targeting people with chronic diseases, with different levels of dependence and who doesn’t have conditions to be cared for at home.
The Unit aims to ensure care that prevent and slow down the deterioration of the situation of dependence, promoting the comfort and quality of life, for a period of hospitalization exceeding 90 consecutive days. This period is lower (90 days per year, usually divided into 3 periods of 30 days each) in the case of admission of users, whose aim is the internment of rest of the caregiver.

How to Access:
If the person is admitted to a Hospital that’s part of the National Health Service (SNS), contact the service at which they interned or the Management Team of High (EGA) of this Hospital.
If you are at home, retire home or another situation , contact the Health Center, through the family doctor, nurse or social worker.

What you should bring
Photocopies of the patients and the informal carer and/or representative`s identification documents.

Articles of personal use of the patients (according to the degree of dependency) - pajamas or nightdress (the most comfortable as possible and preferably of cotton), bathrobe, slippers, underwear, comfortable clothes to wear during the day, tracksuit and sneakers for physical therapy.

Shouldn´t bring
Valuables objects, money, TV, among others, because the Unit will not be responsible for loss or damage of goods considered of value or personal property of the users.

VISITS:
Schedules:
Generalfrom 3pm to 8pm;
For the informal caregiversfrom 9am to 8pm.

Number of visits per patient: 3 people, the caregiver and two more people (visits made by children less than 10 years of age accompanied by an adult and in a short period of time. It´s asked that bringing toys to the visit should be avoid).


Some recommendations

When entering and exiting the room, disinfect hands (with hands disinfection device which is located at the entrance of the rooms), for your safety and of the users;

Always announce your entry and exit of the Unit, at the reception;

Leave the room whenever requested by the team, waiting in the hallway;

During the visit respect the privacy of other users and their companions and the schedules, rules of operation of the Unit and the orientations of the professionals;

Do not give any type of food or drink to the patient, without authorization from the Nurse Responsible;

If you want to mobilize the patient or change the position of the bed you should always request permission from the team of nursing;

If the patient is in a room with restricted access, before entering the room you should speak with the nurses on duty. Depending on the clinical situation of the patient, it may be necessary to wear gloves, dressing gown or put mask. The conditional access shouldn´t be a reason for concern, if all the guidelines given by nursing staff are met.
